Ostara Physiotherapy’s Gold Service Package provides a comprehensive subscription for physiotherapy- ideal for larger homes, homes with frequent falls or homes with regular unplanned hospital admissions. On a monthly basis, this comprehensive subscription includes: * A comprehensive initial assessment and personalised recommendations for five people under your care * One follow up session for individuals reviewed (agreed upon initial consultation) * One bespoke training session for your staff every two months * One online virtual 'check in', to provide further support with regards to preventing falls and promoting safe mobility for the people under your care Examples of service users who would benefit from this service: * People who are falling, or are fearful of falling, and find this to impact their quality of life * People living with dementia who are experiencing changes to their mobility and/or function * People who have developed deconditioning syndrome following discharge from hospital * A person who is new to living with you and would benefit from an assessment of their strength, balance and mobility Note: as with all subscriptions, flexible and optional add-on services can be provided based upon the needs of the people you support. Cancellation Policy
PLEASE NOTE THE FULL TERMS & CONDITIONS WILL BE SHARED ALONGSIDE YOUR CONTRACT. This is a summary relevant to your booking- please take the time to read the full T&C's carefully. 1. SUBSCRIBING - Long term care establishments, including Independent Living Facilities, (all referred to as LTCE's) subscribe to Ostara's services and receive tailored physiotherapy services on a monthly basis 2. PAYMENT Please note that Ostara is a cashless operation where possible - A monthly payment plan will be agreed prior to signing the contract, which may be a readymade package or be tailored to the individual needs of the LTCE - Invoices will be generated by Ostara for the month ahead and must be paid within 30 days of receipt - Non-payment after 30 days will generate a reminder from Ostara and services will cease to continue until payment made. Non-payment remaining 30 days after the final due date will result in Ostara taking action to settle payment 4. CAPACITY & CONSENT - All people receiving services from Ostara must provide informed consent - Any person who lacks capacity to make this decision will be supported to do 5. Cancellations - Upon signing the contract for a new subscription, a 28 day 'cooling off' period applies whereby the purchaser has 28 days to cancel the subscription without notice, however, no refunds can be provided. - Once the 28 day cooling off period has lapsed, a two month cancellation policy applies from both Ostara and the purchaser - Please note the individual sessions as part of a subscription can be rearranged secondary to infection control outbreaks subject to availability- please provide as much notice as possible 6. Travel expenses - Please note that travel expenses are free within 15 miles (as travelled) of the SG12 postcode. Additional mileage will be charged at £2 per mile. Expenses will be added to the following months invoice where relevant.
