Dementia Specialist Initial Appointment
Service Description
During the Dementia specialist physiotherapy initial consultation, Katie will allocate three hours to spend with the person and their support network to build rapport, complete a thorough assessment, and provide personalised recommendations. Katie's aim is to understand the individual's unique strengths, needs and challenges, enabling us to develop a tailored treatment plan that will maintain independence and improve their quality of life. Following the Dementia Specialist Initial Assessment, the client will receive: - A personalised mobility plan, which is shared with the care home - Recommendations for onward signposting and referrals if required - A letter updating their GP of any relevant findings Some clients benefit from further appointments- this can be discussed with the client and their support network- Ostara will only recommend further input if this will be of benefit to the individual.

APPOINTMENTS - Individuals can book Ostara's individual appointments without the need for a subscription - Long term care establishments, including Independent Living Facilities, (all referred to as LTCE's) can add extra services as needed to their existing subscription to receive tailored physiotherapy services on a monthly basis. 2. PAYMENT Please note that Ostara is a cashless operation where possible Payment for this service will be generated via invoice- invoices will be sent by Ostara on a monthly basis and be paid within 30 days of receipt. 4. CAPACITY & CONSENT - All people receiving services from Ostara must provide informed consent - Any person who lacks capacity to make this decision will be supported to do following the principles of the Mental Capacity Act (2005) 5. Cancellations - In the event of a session needing to be rescheduled, ie. due to ill health/hospital admission, please provide as much notice as possible (ideally at least 48hrs) - In the event of a session needing to be cancelled, ie. change of mind, please provide at least 48hrs notice. If 14 days are provided, a 50% return of deposit can be processed. - If able to provide proof of significant ill health preventing further physiotherapy requirements, death (of the client or a loved one) or attendance at a funeral, a full refund of the deposit can be arranged 6. Travel expenses - Please note that travel expenses are free within 15 miles (as travelled) of the SG12 postcode. Additional mileage will be charged at £2 per mile. Expenses will be added to the total amount payable where relevant.
